Under orders from Amanda Waller, imprisoned convicts at Belle Reve penitentiary are sent to the island nation of Corto Maltese to destroy Jotunheim, a Nazi-era prison and laboratory which holds political prisoners and conducted experiments. The Thinker works with the Corto Maltese government as the laboratory's chief scientist, overseeing Project Starfish.
The mission begins with Task Force X members Harley Quinn, Rick Flag, Captain Boomerang, TDK, Mongal, Javelin, Savant, Blackguard, and Weasel deployed onto a Corto Maltese beach in order to journey to the lab. Weasel can't swim and drowns, Blackguard attempts to sell the team out to the Corto Maltase military but is shot and killed. Mongal, Captian Boomerang, TDK, Javelin and Savant are all killed with Harley and Flag being the only survivors. Harley is captured by the army while Flag disappears into the brush. Meanwhile, Bloodsport, Nanaue, Ratcatcher 2, Polka-Dot Man, and Peacemaker arrive on another of the island's beaches and are instructed to find and rescue Flag. The team rescues Flag and convinces the island's rebel faction lead by Sol Soria to join forces with them in order to destroy Jotunheim and remove Corto Maltese president Luna, who has recently come to power in a coup, from power.
Harley is taken to the President's palace to meet Luna and it is revealed that he wants Harley to be his partner. After a short fling, Luna proposes to Harley and explains his plans to use Jotunheim to torture political dissidents. In response, Harley shoots and kills him. She is subsequently arrested and tortured by Luna's men.
Meanwhile, the rest of the Squad stakeout a nightclub, waiting for The Thinker to arrive to use him to infiltrate Jotunheim. Shortly after his arrival, Luna's soldiers raid the club looking for the team. Ratcatcher and Nanaue leave with The Thinker while Flag, Bloodsport and Peacemaker create a diversion by surrendering to the soldiers. While being transported, they are told by one of the soldiers that Harley is alive and being interrogated by Luna's successor, General Suarez. They then manage to escape by killing the soldiers in the truck and crashing the convoy. The Squad reunites and they agree to save Harley before resuming the mission.
Back at the palace, Harley is tortured by the General. She escapes and proceeds to kill the palace's guards, finally exiting and meeting the rest of the team who were on their way into the palace to save her.
The Squad sneaks into Jotunheim and begins to rig the building with explosives while the regime's army surrounds the building. The group splits into two groups to complete their tasks. Harley, Bloodsport, and Polka-Dot Man head for other floors to continue setting explosives. They are attacked by soldiers and their bombs trigger prematurely. As the building begins to collapse, Bloodsport is separated from the rest as he falls down the building with the debris. Flag, Ratcatcher, Peacemaker, and The Thinker, meanwhile, are in the underground laboratory where the subject of Project Starfish, the extra-terrestrial Starro the Conqueror is being held and tortured by The Thinker. The Thinker reveals how gruesome his experiments were, and also how the American government played a large role in the project. Outraged and feeling betrayed, Flag finds the drive with all the project's evidence and threatens to release it to the press. Peacemaker, acting under orders from Waller not to allow the drives to survive, draws a gun on Flag. However, the prematurely-triggered bombs cause the laboratory ceiling to collapse and engulf them. A fight between Flag and Peacemaker ensues. As the debris settles, Starro is released by the destruction and kills The Thinker. Ratcatcher runs and witnesses Peacemaker killing Flag. Peacemaker sees her take the drive and chases her down. As he is about to shoot Ratcatcher, Bloodsport falls from the crumbling debris and shoots Peacemaker.
The surviving Squad reunites just as Starro breaks out of the laboratory. Terrified, the team runs out of the ruined building while Starro releases spores that attach themselves to the soldiers' faces, controlling them. Starro head towards the city and wreaks havoc. Waller contacts the team, informing them the mission is successfully over as they have destroyed the lab. They watch as Starro destroys the city and refuse Waller's orders to stand down, instead deciding stop Starro from killing more people. The remaining team; Harley, Bloodsport, Polka-Dot Man, Nanaue, and Ratcatcher all work together to take down Starro. Polka-Dot Man dies in the fight while Harley has a close call. In a final and desperate attempt, Ratcatcher summons the city's rats to overpower Starro and his mind-controlled minions and it is finally taken down. Relieved, the team regroup and Bloodsport blackmails Waller with the drive, demanding she leaves them to be free.
A mid-credits scene reveals Weasel to be alive, another credit scene reveals a wounded Peacemaker in the hospital. Waller's team visits him and hints at more missions.
